ABSTRACT
PURPOSE : To evaluate and compare clinical inflammatory responses to the surgical trauma caused by cholecystectomy via several access approaches: single-port umbilical incision (SILS), transvaginal natural orifice transluminal endoscopic surgery (NOTES), laparoscopy, Laparotomy. METHODS Twenty-eight female pigs were equally divided into four groups submitted incision, NOTES, or An additional five animals served as controls (sham group). Animals monitored perioperatively regarding anesthesia procedure times, well for presence of complications. Postoperatively, they evaluated time ambulation feeding, events. Procalcitonin, C-reactive protein (CRP), AQUI feron-gamma (IFN-γ) measurements performed before immediately, two days, seven days after surgery. sacrificed necropsied at RESULTS All procedures successfully proposed in each group. Only minor complications, such gallbladder perforation bleeding from liver bed, observed during all groups. The vaginal NOTES group showed higher times compared other (p<0.001). No between-group differences perioperative postoperative evolution, serum markers observed. adhesions found on necropsy, with no between CONCLUSION: approaches feasible safe laparoscopic laparotomy cholecystectomy.
