Opportunities to improve the effectiveness of complex therapy for endometriosis and chronic pelvic pain in patients of reproductive age

ABSTRACT
Pain is the most frequent and specific symptom of endometriosis. Objective: To increase the effectiveness of complex therapy of endometriosis patients suffering from chronic pelvic pain. Materials and methods of research. A comparative analysis of the results of examination and treatment of 34 women (18—45 years old) with a histologically verified diagnosis of endometriosis was carried out. The patients were divided into 2 groups: 1 — 12 patients with endometriosis without pain syndrome, 2 — 22 with chronic pelvic pain and endometriosis. Group II is divided into subgroups: 2A — 11 women with chronic pelvic pain, 2B — 11 — with the presence of a neuropathic component of pain syndrome. All patients received dienogest 2 mg day. Group 2A patients additionally received Amitriptyline 12.5 mg at night, and group 2B patients received Clonazepam at a dosage of 2 mg per day at night and non-drug treatment methods. The results obtained. Against the background of ongoing therapy among patients taking Amitriptyline and Clonazepam, the severity of chronic pelvic pain in group 2 decreased by 8 points. The quality of life in group 1 patients increased by 50% by the end of 3 months of treatment, in group 2A patients by 53%, in group 2B patients by 60%. Conclusion. For effective treatment of endometriosis and chronic pelvic pain, an interdisciplinary approach needs to be developed. The drugs Clonazepam and Amitriptyline in combination with non-drug methods of analgesic therapy have demonstrated effectiveness in relieving chronic pain. Study design: comparative prospective.
