ABSTRACT
Abstract Endocardial radiofrequency ablation of the septal hypertrophy (ERASH) is an alternative for hypertrophic obstructive cardiomyopathy (HOCM). Radiofrequency could lead to regional myocardial akinesia, so as relieve left ventricular outflow tract (LVOT) gradient. We reported that two patients presented Left asymmetric in transthoracic echocardiography, and were diagnosed with HOCM. One had a gradient up 210mmHg during provocation. The other LVOT 108mmHg at rest. Both are symptomatic despite optimized medication. Due restriction coronary anatomy, lost opportunity alcohol (ASA). ERASH programmed. target site was determined by intracardial echocardiography electroanatomic Carto-mapping system. caused targeted myocardium hypokinesia precisely, aiming interdicting mitral valve systolic anterior motion. 1-month follow-up showed remarkable improvement clinical state predominant reduction assessing echocardiography. comes new solution patient who not candidate myectomy ASA. system intracardiac enable precise lesion placement preservation atrioventricular conduction.
