ABSTRACT
<sec> <title>BACKGROUND</title> Care for mild to moderate chronic kidney disease (CKD) entails self-management from patients and clinical support primary care nephrology. In response a gap in resources this population, My Kidneys Health was co-developed of CKD. While is patient-facing tool, health providers play critical role the implementation patient </sec> <title>OBJECTIVE</title> This study develops evaluates strategies implement into routine general nephrology care. <title>METHODS</title> working Alberta, Canada who with CKD were invited participate our multi-step study, guided by Quality Implementation Framework. Step 1: we followed qualitative descriptive methodology identify barriers enablers using directed content analysis deductive coding approach. Participants complete semi-structured interviews October 2021 May 2022. 2: identified, prioritized, co-developed, launched based on behaviour change theory. 1 use materials during period (May 2022). Utilization tracked through Google Analytics document distribution tracking. 3: conducted follow-up participants (October December 2022) evaluate Reach, Effectiveness, Adoption, Implementation, Maintenance framework, following same approach as 1. Effectiveness out scope study. <title>RESULTS</title> 14 participated (42.9% clinics, 35.7% care). shared an individual-level readiness interest sharing their patients.
