Predictive Factors Involved in Determining Response to Neoadjuvant Chemotherapy in Breast Cancer and Impact of Response on 5 Years Disease Free Survival and Overall Survival

ABSTRACT
Objective: To evaluate all the known factors that may play a role in predicting response to Neoadjuvant chemotherapy breast cancer and see impact of on five years’ disease free survival (DFS) Overallsurvival (OS). Material Method: Data 156 patients was reviewed retrospectively from January 2012 December at Shaukat Khanum Memorial Cancer Hospital Research Centre Lahore, Pakistan. All received neoadjuvant (NAC) had no distant metastasis. The measured term percentage reduction 1st radiological size presentation final histopathology (of resected specimen). Four groups were identified, complete responder (CR) (100% reduction), Responders (R) (>50% Partial (PR) (<50% reduction) Non-responder (NR). Relationship predictive with each group observed. Five year noted for group. Result: Median age 45 years (25-64 years). 67% underwent conservation surgery, while rest mastectomy. Mortality whole 22%, recurrence shown 34% (Majority i.e. 26% distant, contralateral 3%). Out patients, 25% CR, 13% NR, 23% PR 37% R. Progesterone receptor negative Grade III tumors showed more responses. Rest types, including triple negative, initial T N stage other clinical chemo-response. Survival significantly poor NR (45% OS, 40% DFS), three comparable outcome, CR having best outcome (86% 80% DFS). Conclusion: Most studied did not show achieving good chemo response, however better survival.
