ABSTRACT
The interaction between killer immunoglobulin-like receptors (KIRs) and HLA class I regulates natural (NK) cell cytotoxicity function. impact of NK alloreactivity through KIR in liver transplantation remains unelucidated. Since the frequency HLA-C genotypes show ethnic differences, we assessed HLA-C, genotype, or KIR-ligand mismatch on allograft outcome Korean allografts.One hundred eighty-two living donor transplant patients were studied. Thirty-five (19.2%) had biopsy-confirmed acute rejection (AR), eighteen (9.9%) graft failure. compatibility, genotypes, ligand-ligand, matching was retrospectively investigated for association with outcomes.Homozygous C1 ligands predominant both donors, HLA-C2 allele Koreans lower than that other groups. Despite significantly genotype Koreans, donors at least one showed higher rates AR no alleles (29.2% vs 15.7%, P=0.0423). Although also number activating KIR/inhibitory not associated outcome. expected 31.6% transplants survival.This study could confirm clinical mismatch. However, demonstrated presence influenced allografts.
