High-Flow Nasal Cannula Oxygen Therapy Can Be Effective for Patients in Acute Hypoxemic Respiratory Failure with Hypercapnia: a Retrospective, Propensity Score-Matched Cohort Study

ABSTRACT
Usually, high-flow nasal cannula (HFNC) therapy is indicated for de novo acute hypoxemic respiratory failure (AHRF). Although only a few researches have examined the effectiveness of HFNC with hypercapnia, this often performed under such conditions various reasons. We investigated AHRF patients hypercapnia compared to those without hypercapnia.All consecutive receiving between January 2012 and June 2018 at university hospital were enrolled classified into nonhypercapnic hypercapnic groups. outcomes both groups adjusted propensity score matching.A total 862 enrolled, which 202 included in group. weaning success rates higher, intensive care unit (ICU) mortality was lower group than (all P < 0.05). However, no statistical differences (adjusted = 0.623, matched 0.593), ICU 0.463, 0.195), 0.602, 0.579) noted from propensity-adjusted propensity-matched analyses. Additionally, score-matched subgroup analysis (according chronic lung diseases causes application), there also significant difference two groups.In underlying conditions, might be helpful hypercapnia. Large prospective randomized controlled trials are required firm conclusions.
