ABSTRACT
Background/Aims The aim of this study was to identify the parameters that could noninvasively predict presence esophageal/gastric varices and portal hypertensive gastropathy (PHG) in patients with chronic liver disease (CLD), determine accuracy those parameters. Methods We retrospectively analyzed 232 CLD who underwent both upper endoscopy CT within an interval 3 months. multidimensional index (M-Index) for spleen volume obtained from multiplication splenic length, width, thickness, as measured by computer tomography. Results multivariate analysis revealed platelet, albumin, M-Index were independently associated PHG. combined three independent parameters, developed a (VAP) scoring system (=[platelet count (/mm3)×albumin (g/dL)]/[M-Index (cm3)]). area under receiver operating characteristic curve VAP score 0.850 (95% confidence interval, 0.801-0.899). cut-off value 861 had sensitivity 85.3%, positive likelihood ratio 3.17, negative predictive 86.4%. For predicting high-risk lesions bleeding, 92.0%, 2.20, 96.4%. Conclusions can PHG may increase cost-benefit screening clinical practice setting. A prospective validation is necessary future.
