ABSTRACT
Background The red blood cell distribution width (RDW)-to-albumin ratio (RAR) has emerged as a potentially valuable prognostic indicator in diverse medical conditions. However, the significance of RAR intensive care unit (ICU) patients with coronary heart disease (CHD) and diabetes mellitus (DM) remains uncertain requires further investigation. Methods This study aims to investigate ICU coexisting CHD DM through retrospective cohort analysis using data from Medical Information Mart for Intensive Care IV (MIMIC-IV) database (version 2.2). population included aged 18 years or older who were diagnosed both DM. primary endpoint was 1-year mortality, secondary endpoints 30-day 90-day hospital length stay (LOS), LOS. Results A total 3416 patients, whom 64.64% male, study. mortality 7.08%, 7.44%, 7.49%, respectively. After adjusting confounding factors, multivariate Cox proportional risk demonstrated that high levels associated an increased (HR, 1.53 [95% CI 1.17-2.07], P = 0.006), 1.58 1.17-2.13], 0.003), 0.003). Furthermore, restricted cubic spline (RCS) model indicated linear relationship between mortality. Conclusion results suggest holds potential biomarker Elevated found be significantly during hospitalization, facilitating identification individuals at higher adverse outcomes. These findings underscore importance incorporating into stratification overall management strategies
