Multicenter Performance Evaluation of MALDI-TOF MS for Rapid Detection of Carbapenemase Activity in Enterobacterales: The Future of Networking Data Analysis With Online Software

ABSTRACT
In this study, we evaluate the performance of matrix-assisted laser desorption/ionization time-of-flight mass spectrometry (MALDI-TOF MS) for rapid detection carbapenemase activity in Enterobacterales clinical microbiology laboratories during a multicenter networking validation study. The study was divided into three different stages: “software design,” “intercenter evaluation,” and “clinical validation.” First, standardized procedure with an online software data analysis designed. Carbapenem resistance detected by measuring imipenem hydrolysis results were automatically interpreted using Clover MS (Clover BioSoft, Spain). Second, series 74 genotypically characterized (46 carbapenemase-producers 28 non carbapenemase-producers) analyzed 8 international centers to ensure reproducibility method. Finally, methodology evaluated independently all 2-month period compared reference standard used each center. overall agreement rate relative method samples 92.5%. sensitivity 93.9% specificity, 100%. Results obtained within 60 min accuracy ranged from 83.3 100% among centers. Further, our demonstrate that MALDI-TOF is outstanding tool laboratories. use simple in-house allows routine screening carbapenemases diagnostics, thereby facilitating early appropriate antimicrobial therapy.
