Thoracic endovascular aortic repair (TEVAR) of acute uncomplicated type B dissection (uTBAD) has been discussed for its potential to prevent future events. We present a fenestrated TEVAR in the case an 86-year-old man with uTBAD isolated left vertebral artery (ILVA). The ILVA originated from distal side subclavian artery, and intramural hematoma ulcer-like projection extended close artery. selected stent graft achieve proximal healthy landing. This demonstrates that is useful preserving arch vessels.
