ABSTRACT
<br><b>Background:</b> Ventilator-associated pneumonia (VAP) is the most frequent intensive care unit (ICU)-acquired infection. The etiology of VAP and their antimicrobial susceptibility pattern varies with different patient populations types ICUs. <b>Materials Methods:</b> An observational cross-sectional study was performed over a period 2 years in tertiary hospital to determine various etiological agents causing detect presence multidrug-resistant (MDR) pathogens these patients. Combination disk method, Modified Hodge test, ethylenediaminetetraacetic acid synergy AmpC test were for detection extended-spectrum beta-lactamase (ESBL), carbapenemases, metallo-beta-lactamases (MBL), beta-lactamases, respectively. <b>Results:</b> prevalence 35%. <i>Enterobacteriaceae</i> (66.66%) <i>Staphylococcus aureus</i> (20%) common early-onset VAP, while nonfermenters (50%) (40.61%) predominant from late-onset VAP. Nearly 60.87% bacterial MDR. ESBL produced by 21.74% <i>Enterobacteriaceae</i>. β-lactamase positive 35.29% 26.08% MBL 17.64% 17.39% Among <i>S. isolates, 75% cefoxitin resistant. Prior antibiotic therapy (<i>P = 0.001)</i> hospitalization 5 days or more (<i>P</i> 0.001) independent risk factors MDR pathogens. polymyxin B, tigecycline, vancomycin sensitive drugs Gram-negative isolates respectively <b>Statistical Analysis:</b> SPSS Windows Version 17.0 (SPSS Inc., Chicago, IL, USA) Chi-square Yates correction. <b>Conclusion:</b> Late-onset increasingly associated Treatment should be kept as last-line reserve against pathogens.<br>
