Endogenous Endophthalmitis Caused by Aspergillus lentulus in an Immunocompromised Patient with Lung Cancer

ABSTRACT
A 78-year-old man with a history of lung cancer, chemotherapy, radiotherapy, and coronavirus disease 2019 infection experienced visual deterioration two-weeks’ duration in his right eye. There was multifocal, yellowish-white retinitis foci, vascular engorgement, scattered intraretinal hemorrhages extending from posterior pole to retinal periphery the eye, whereas left eye normal. Intravitreal vancomycin, ceftazidime, clindamycin, dexamethasone were given for endogenous endophthalmitis initially. Vitreous culture confirmed presence Aspergillus lentulus, he treated intravitreal amphotericin-B voriconazole injections together systemic amphotericin-B, voriconazole, posaconazole, micafungin therapy. During follow-up, vitreoretinal surgery performed because rhegmatogenous detachment, received one additional cycle chemotherapy due recurrence cancer. Although retina attached, enucleation eventually required painful red Atypical squamous cells beneath neurosensory suggesting metastasis noted on histopathological examination. Timely ocular examination is crucial any immunocompromised patient having symptoms. High level suspicion fungal etiology must these patients.
