This concept article introduces a transformative vision to reduce the population burden of chronic disease by focusing on data integration, analytics, implementation and community engagement. Known as PHOENIX (The Population Health OutcomEs aNd Information EXchange), approach leverages state level health information exchange multiple other resources facilitate integration clinical social determinants with goal achieving true monitoring management. After reviewing historical context, we describe how multilevel multimodal can be used core public services, before discussing controversies challenges that lie ahead.
