ABSTRACT
A 71-year-old male with severe obstructive sleep apnea and nasal septal deviation presented to a positive airway pressure (PAP) alternatives clinic due persistent events on both continuous PAP bilevel therapy delivered via oronasal mask. He underwent drug-induced endoscopy titration determine the mechanism of mask failure. was also applied titrated for comparison. Drug-induced showed tongue base collapse which resolved at low using Application an increased minimum therapeutic level. Tightening worsened collapse, not by increasing Following surgery, patient able tolerate pressure, resulted in objective self-reported improvement is his apnea. This case highlights ability mechanistic cause
