[A quality improvement study on improving the follow-up rate of preterm infants after discharge].

ABSTRACT
To explore the measures to improve follow-up rate of preterm infants after discharge, and evaluate effectiveness these using quality improvement methodology. The status discharged from March May 2017 was used as baseline before improvement, a specific goal for proposed. Pareto chart analyze causes failure, key driver diagram constructed based on links involved in improving rate. failure were analyzed determine intervention monitored weekly control until achieved. discharge 57.92% (117/202) at set increase more than 80% within 12 months. analysis showed that main deficiencies file management irregular times (33.70%, 31/92), insufficient education poor communication (25.00%, 23/92), inability meet diverse needs parents (18.48%, 17/92). Based following adopted: (1) strengthen publicity education; (2) build team; (3) establish platform system. indicated with implementation above measures, increased 74.09% (306/413) July 83.09% (511/615) December 2017, finally achieving goal. During COVID-19 pandemic, fluctuated between 23.54% (460/1 954) 70.97% (1 931/2 721), subsequently, it returned pre-pandemic levels starting February 2023. application methodology can help formulate thereby discharge. This method is feasible practical thus holds promise clinical application.
