We herein report a case of bilateral pneumothorax after unilateral transbronchial lung cryobiopsy (TBLC). A 73-year-old man with no history cardiothoracic surgery underwent TBLC for the reevaluation interstitial disease. Five hours later, he developed pneumothorax, pneumomediastinum, and subcutaneous emphysema. He chest drainage was discharged 18 days later. The biopsy specimens obtained from contained visceral pleura bronchial cartilage, suggesting injury as cause pneumothorax.
背景．肺癌による卵巣転移の頻度は低いが，卵巣腫瘍内への腫瘍内転移はさらに稀である．症例．79歳女性．1ヶ月間続く咳嗽を認め，精査目的のために当院へ紹介．気管支鏡検査にて肺癌と診断された．卵巣腫瘍も併存していたがPET/CTでFDGの異常集積がないため，良性腫瘍と判断した．肺癌に対し外科的肺切除術を施行し，肺大細胞神経内分泌癌（large-cell neuroendocrine carcinoma；LCNEC），stage IIIA期（pT3N1M0）であった．術後13ヶ月からPro-GRPの上昇を認め，PET/CTで卵巣腫瘍内にFDGの集積亢進を認めた．原発性卵巣癌が疑われたため，子宮全摘，両側卵管卵巣摘出および大網切除術を施行した．卵巣腫瘍内に紡錘形細胞と類円形細胞が混在しており，紡錘形細胞は卵巣線維腫と診断した．類円形細胞は，肺癌切除検体に類似し，神経内分泌マーカー陽性であり，LCNECによる卵巣腫瘍内転移と診断した．結論．LCNECの卵巣腫瘍内転移を経験した．良性腫瘍が併存するが，腫瘍マーカーが上昇するなど再発が示唆される症例には腫瘍内転移も考慮すべきである．
