症例は60代女性.労作時呼吸苦,右季肋部痛を主訴に受診され,CTで多発巨大肝囊胞による胸腔の圧排が指摘されたため当科紹介となった.最大径14 cmの肝囊胞を10個肝臓内に認め,同時に脾腫瘤も指摘された.3年前には肝囊胞は指摘できず,悪性腫瘍の可能性を考えEUS,CT,MRI,PET-CT,上下部内視鏡検査,造影エコー検査を施行し,囊胞穿刺で囊胞内の液体を細胞診に提出したがいずれの検査でも悪性所見は認められなかった.臨床経過から悪性腫瘍と判断し,他院にて拡大肝右葉切除術を施行.囊胞壁は線維性被膜様組織からなり,内部で異型細胞が血管様の腔を形成しながら増殖していた.核異型が目立ち,CD31(+),CD34(+)であることから病理学的に血管肉腫と診断した.単純性肝囊胞の画像所見を呈する血管肉腫の報告はなく,特異な臨床経過を辿ったため報告する.

73歳，女性。出生時より腰背部に扁平な黒色斑があり，1週間に1～2回ドライアイスで凍結療法をしていた。瘢痕が目立つようになり10歳で中断。約3年前から背部に瘙痒を伴う皮疹が出現し精製白糖・ポピドンヨード配合軟膏を外用されるも改善せず。背部から腰にかけて約50cm大の境界明瞭な灰青色から褐色の色素斑を認め，その上に白色調の瘢痕が散在し，4個の結節と2個の紅斑。結節の内訳は辺縁が堤防状に隆起し中央に潰瘍を伴う5cm大の結節，辺縁が点状の黒色斑で縁取られた2cm大の扁平隆起性結節が2個，2mm大の黒褐色結節で，いずれも弾性硬で下床との可動性良好。組織は典型的な基底細胞癌。さらに類円形から不整形の鱗屑の付着した紅斑が2個あり，組織はBowen病。腫瘍辺縁の色素斑の真皮内に母斑細胞が孤立性に散在。腫瘍は全て切除し断端陰性で半年で再発なし。これまでに雪状炭酸圧抵療法による基底細胞癌とBowen病の併存報告はなく，既報告18例をまとめた。
