Traumatic rupture of the right hemidiaphragm occurred following a high-velocity motor vehicle collision in case presented herein. The resulting herniation small bowel loops into thorax resulted hemodynamic and respiratory compromise due to pressure effects on heart major vessels. patient's status improved with reduction enterothorax, diaphragmatic defect was repaired. We discuss available literature learning points from this rare case.
