Leiomyosarcoma of the rectum is a rare neoplasm and may behave in highly malignant manner. Our experience with single case prompted review Armed Forces Central Medical Registry (AFCMR), where six other cases were found from total AFCMR population. Four patients treated abdominoperineal (AP) resection; three had local excision only. Three developed metastatic disease two those died. The authors recommend prompt wide (AP resection) for any leiomyosarcoma rectum.
