Tumefactive multiple sclerosis (MS) は広範な浮腫や巨大な病変を形成することから, 脳腫瘍と鑑別が困難な例が多い. 本症例は66歳男性で右上下肢の単純部分痙攣発作で発症した. 左前頭葉の病変は画像上悪性グリオーマが疑われ摘出術が行われたが, 病理検査で脱髄性の所見や広範な出血および壊死像, Creutzfeldt cellを認めたことからtumefactive MSの診断に至った. 診断には病理検査が決定的となるが, 画像上病変部の血流上昇を認めないことが悪性グリオーマとの鑑別点と考えられた.

Case Description: A 77-year-old woman was initially diagnosed with unruptured cerebral aneurysm at the right basilar artery-superior junction by magnetic resonance angiography.
