We present a case of 67-year-old patient who significantly benefit from conduction system pacing. The presence septal scar initially limited chance success. Ineffective attempt to upgrade the ICD CRT along with end stage heart failure despite optimal medical therapy, however, forced us make an existing CSP. procedure itself was demanding, but after numerous attempts we achieved satisfactory electrocardiographic effect.
