OBJECTIVES:Expert consensus jointly authored in 2011 by the American Academy of Pediatrics, Family Physicians, and College Physicians supports use health care transition (HCT) coordination (CC). Although gaps are addressed these practice-based implementation recommendations, such recommendations have never undergone rigorous assessment.
