症例は33歳,ラテンアメリカ系の女性.母の従妹に乳癌家族歴があった.1カ月前からの右乳房腫瘤を主訴に,前医を受診した.右乳頭直下に8cm大の腫瘤性病変を指摘され,針生検を施行された.生検の結果,悪性の所見はなかったが,巨大腫瘤のため手術目的に当科へ紹介となった.術前画像診断では,内部に多数の嚢胞性変化を伴う75mm大の腫瘤性病変がみられた.当院での生検組織の見直しでductal carcinoma in situを完全に否定するのが困難な所見がみられたものの,画像上は明らかな悪性所見はなく,右乳腺腫瘤摘出術が施行された.術後病理にて,若年性乳頭腫症(juvenile papillomatosis)の診断に至った.若年性乳頭腫症は稀な疾患であり,アジア人での報告は少ないとされる.治療は完全切除であり,悪性腫瘍の合併がなければ追加治療は不要である.しかし,乳癌家族歴などがある場合は乳癌発症のリスク因子となる可能性が示されており,術後も定期的なフォローアップが必要となる.
