【症例】70歳台,女性.【現病歴】上行結腸癌,多発肝転移,肺転移の診断で結腸右半切除術を施行した.病理はpT3pN2b cM1b pStageIVb,RAS野生型,BRAF V600E変異型であった.Bevacizumab+FOLFOXIRI療法を開始したが,Microsatellite instability-high(以下,MSI-H)であったため,Pembrolizumabに変更した.肺転移は消失,肝切除を施行した.病理はpCRで癌細胞は認めなかった.【考察】BRAF変異陽性切除不能大腸癌の1-2次治療不応後化学療法としてBRAF阻害薬を用いたEncorafenib±Binimetib+Cetuximab併用療法で既存治療より生存期間の延長を認める報告があるが,MSIを測定することで予後良好群を選別し,免役チェックポイント阻害薬による個別化医療ができる可能性がある.

症例は65歳，女性．検診で便潜血陽性を指摘され，近医で下部消化管内視鏡検査を施行したところ肛門縁近傍の直腸後壁に約３cm大の粘膜下腫瘍を認め当科紹介となった．超音波内視鏡下に穿刺吸引生検を施行しgastrointestinal stromal tumor（GIST）と診断した．根治性を考え腹腔鏡下直腸切断術を提示したが，患者本人の強い肛門温存希望があったため，十分なインフォームドコンセントのもとでメシル酸イマチニブでの術前補助療法を施行した．投与開始後明らかな副作用は認めず，８週間後に約50％まで腫瘍縮小が得られた．計12週間投与した後に経仙骨的直腸切除術を施行した．GISTに対する術前補助療法は臓器温存や手術侵襲の低減に有用であり，今後治療法選択肢のひとつとなると考えられた．
