症例は70歳，男性。主訴は腹部膨満感と腹痛。朝出勤途中に突然の腹痛と腹部膨満感を訴え，近医を受診。受診時血圧80/50mmHgと低値であり，超音波検査にて腹腔内出血を疑われ当院へ救急搬入された。緊急腹部造影computed tomography（CT）にて膵頭部背側に血腫と思われる腫瘤影を認め，3 dimension-computed tomography（3D-CTA）にて正中弓状靭帯によって圧迫され狭窄した腹腔動脈起始部が認められた。緊急腹部血管造影にて膵十二指腸動脈に動脈瘤を認めた。これらの所見より正中弓状靭帯圧迫症候群に伴う膵十二指腸動脈瘤とその破裂と診断した。マイクロコイルにて動脈瘤の塞栓術を施行し，術後経過は良好であった。膵十二指腸動脈破裂による出血に対する緊急治療は，安全性の面から塞栓術を第一選択と考える。動脈瘤の成因と考えられる正中弓状靭帯圧迫について，その圧迫解除の手術適応につき今後検討する必要がある。

症例は79歳の男性で，約40年前に膵体尾部の囊胞性疾患に対して開腹術の既往があるが詳細不詳である．上部消化管内視鏡検査で胃体後壁からの出血を認め，囊胞内出血からの穿破が疑われた．緊急血管造影で脾動脈から膵囊胞内出血，胃穿破を認め，脾動脈本幹を塞栓し止血をえた．全身状態の改善を待って待機的に手術施行した．膵囊胞病変周囲は炎症性変化で強固な癒着を来していたため，胃上部，膵体尾部，脾臓，結腸脾彎曲部，左副腎を一塊として摘出した．病理組織学的検査では囊胞壁は肉芽組織，膿瘍で覆われ仮性囊胞であった．膵組織は萎縮しほぼ消失していたが，胃囊胞瘻孔部対側の囊胞壁外の萎縮した膵臓に腺癌を認めた．本症例は長期間経過観察されていた膵仮性囊胞に併存した膵癌と考えられ，炎症をくり返し癌化した可能性もあり貴重な症例と考え報告する．
