症例は57歳男性である.HIV(human immunodeficiency virus)に対するcART(combination antiretroviral therapy)中に認知機能低下が出現し,頭部MRIで両側大脳白質や脳幹部に病変をみとめた.髄液中HIV-RNAが血中HIV-RNAよりも高値を示し,薬剤耐性検査で抗HIV薬への耐性が判明.薬剤耐性と中枢神経移行性を考慮して抗HIV薬を変更し,認知機能の改善と髄液中HIV-RNAの陰性化がえられた.抗HIV薬の選択にあたっては,中枢神経移行性を考慮することが重要である.
感覚発作から焦点意識減損強直発作,ついで焦点起始両側強直間代発作に進展した若年男性の2症例を経験した.発作型は片側上肢の異常感覚から上下肢が伸びる強直姿勢をとり,焦点起始両側強直間代発作に至った.異常感覚や強直姿勢は帯状回皮質への皮質電気刺激で報告があり,感覚発作や焦点意識減損強直発作を認める場合には帯状回由来の発作を疑う必要がある.2例とも頭部MRIで帯状回に異常信号を認め,血液中の抗ミエリンオリゴデンドロサイト糖蛋白(myelin oligodendrocyte glycoprotein,以下MOGと略記)抗体陽性が判明し,抗MOG抗体陽性大脳皮質脳炎と診断した.ステロイド治療が有効だった経過から抗MOG抗体関連疾患による帯状回病変が発作の焦点となったと考えた.

後脊髄動脈領域の梗塞は脊髄梗塞の中でも稀であり,延髄下部から上位頚髄の背側領域の障害を来すことがある.延髄下部では三叉神経脊髄路核と楔状束は解剖学的に近接しているが,今回後脊髄動脈領域の梗塞により両部位が障害され,特異的な感覚障害を呈した症例を経験した.症例は74歳,男性.突然の上肢優位の左上下肢の深部感覚障害とonion-skin patternの左顔面感覚障害を呈した.MRIで延髄下部から上位頚髄の梗塞を認め,MRAとCT-angiographyの所見から左椎骨動脈解離が疑われた.体性感覚誘発電位で左後索障害を示す所見を認めた.アスピリンの内服とリハビリテーションを継続し,発症3カ月後には左上肢の深部感覚障害と左顔面の感覚障害の改善を認めた.顔面の異常感覚と同側の上肢の深部感覚障害は,延髄下部から上位頚髄の病変で起こり,通常の頭部MRI撮影で見逃しやすい部位であり,注意が必要である.
