Background Chest pain is one of the most common presentations to a hospital, and appropriate triaging these patients can be challenging. The HEART score has been used for such purposes in some countries only few validation studies from USA are available. We aim determine utility presenting with chest an inner-city hospital USA. Patients methods retrospectively screened 417 consecutive admitted observation/telemetry units at Einstein Medical Center Philadelphia. After applying inclusion...

Iliopsoas injury a relatively uncommon condition that may with hip or groin pain. It usually seen in athletes due trauma, particularly flexion injuries. However, spontaneous iliopsoas tendon tear extremely rare, only small number cases have been reported; it has an estimated prevalence 0.66% individuals from 7 95 years. Dosing cefepime for renal function does not completely prevent neurotoxicity in a kidney transplant patient. Cefepime has been reported primarily among patients with insufficiency who received standard doses of the antibiotic. We report case nonconvulsive status epilepticus from dose-adjusted The timing symptoms along clinical and electroencephalographic improvement after discontinuation was critical to diagnosis. Whether we should adjust dose differently patient transplanted is unknown.

Lactobacilli are Gram-positive anaerobic rods or coccobacilli, commonly found as commensals in human mucosa. Rarely, they can cause serious infections such infective endocarditis (IE), and the most frequently implicated species causing L. casei rhamnosus . IE caused by Lactobacillus jensenii is very rare, with only six reported cases so far, to best of our knowledge. We present a case native aortic valve , complicated root abscess complete heart block, requiring emergent surgical intervention.

Incidence of infective endocarditis during pregnancy is around 0.006% with high maternal and fetal mortality. Bacillus cereus an extremely rare cause for in intravenous drug abusers (IVDA) or those valvular disease devices such as pacemakers. We report a case B. endocarditis, which, to the best our knowledge, has never been reported pregnancy. A 30-year-old, 25-week pregnant female presented right shoulder pain, swelling erythema on lateral aspect deltoid muscle from large abscess over her...

Elevation in cardiac troponins is common with sepsis despite unclear impact. We investigated whether demand ischemia(DI) resulted variable outcomes compared to acute myocardial infarction(AMI) and those neither DI nor AMI sepsis. We analyzed data from the 2011-2014 National Inpatient Sample among patients admitted for sepsis. We i) versus ii) AMI, respectively using propensity matching. Primary study end-point was in-hospital mortality. We studied 666,154 patients, mean age 63.7 years 50.8%...

Nonbacterial thrombotic endocarditis (NBTE) is described in patients with mucin-producing cancers and connective tissue disorders (usually SLE). We report NBTE the setting of primary antiphospholipid antibody syndrome (APS). Objective: Thyroid storm is a rare life-threatening medical emergency. It associated with high mortality (10 to 30%). Though there may be mild derangements in liver function tests (LFTs), acute failure (ALF) infrequently reported and its pathophysiology poorly understood. We hereby report case of ALF patient thyroid storm. Methods: A 41-year-old female history Graves disease (non-compliant treatment) complicated by multiple episodes atrial fibrillation (Afib) presented exertional dyspnea...

Abstract Background Accurately assessing volume status in acutely decompensated heart failure (ADHF) can be challenging. Inferior vena cava (IVC) dynamics by echocardiography allow indirect assessment of these patients. Recently introduced hand‐held ultrasound devices are promising. We aimed to describe the clinical correlates using a device ADHF. Methods In this prospective study, we evaluated 106 patients admitted with First scan was performed within 24 hours admission and timed reference...

BACKGROUND:Most abdominal cysts, including adrenal pseudocysts, are benign and asymptomatic. Rapid enlargement, hemorrhage, infection, rupture with leakage of cyst contents, or pressure on adjacent organs can cause symptoms. Although usually diagnosed incidentally imaging, determining the origin a sometimes be challenging. In these situations, surgical excision pathological analysis is crucial to diagnosis management. We report here case giant symptomatic pseudocyst that closely mimicked...
