Prostate ductal adenocarcinoma, a rare histology observed in 0.4-0.8% of all prostate cancers, is treated similarly to acinar adenocarcinoma but tends have higher likelihood metastasis, recurrence, and poorer prognosis.
症例は50代男性．右陰囊違和感を自覚し前医初診した．胚細胞腫瘍マーカーは正常値であったが右精巣腫瘍の診断で右高位精巣摘除術を施行された．組織診断では粘液産生を認める腺癌が疑われ免疫染色ではCK，CAM5.2，CEA，CA19-9，CK7，CK20陽性であった．病理結果をふまえた血液検査でCEAとCA19-9は高値を認めた．全身検索の結果，傍大動脈リンパ節の腫大を認め後腹膜リンパ節郭清施行し血清CEAは正常化した．術後4カ月後に亀頭腹側に疼痛を伴う結節が出現し血清CEAも再上昇したため陰茎全摘術を施行，病理結果は精巣腺癌の転移でありCEAは正常化した．術後9カ月後のCTで肺への転移が疑われCEA再上昇も認めたためTIP（パクリタキセル，イホマイド，シスプラチン）療法を開始した．2コース施行後のCTで肺転移は消失，血清CEAも正常化したが5カ月後の胸部CTで新たな肺転移を認めた．現在，高位精巣摘除後2年以上経過し生存している．
