要旨:76歳女性の右腎盂癌患者に対して後腹膜到達法による腹腔鏡下腎尿管全摘除術を施行した.二酸化炭素による気腹をした300分間, 血圧, 体温, 酸素飽和度は安定していたが終末呼気二酸化炭素分圧(ETCO2)は徐々に上昇し55mmHgのピークに達した.血液ガスは急性呼吸性アシドーシスの所見であった.ETCO2の上昇に伴って調節呼吸の換気回数を増加させたがPCO2の上昇を防ぎ得なかった.手術部位から頚部, 顔面にかけて皮下気腫を認めた.手術終了後のポータブル胸部撮影により, 縦隔気腫および頚部から胸壁に広がる広範囲な皮下気腫を認めた.喉頭鏡による観察を行ったところ喉頭浮腫のための声帯が直視できない状態であった.患者の気道は著明な喉頭浮腫のため閉塞していることが強く疑われた.挿管チューブのカフ注入物を抜いてリーク音を聴取したが確認できなかったため, 喉頭浮腫が高度であると推定され, 即日抜管を見合わせた.我々は術中に顔面頚部に皮下気腫がある場合には, 喉頭浮腫に伴う気道閉塞を回避するため抜管前に喉頭鏡検査を行うべきであると推奨する.
