Chest wall resection following wide local excision for bone tumor results in a large defect. Reconstructing this defect is complex and requires skeletal soft tissue reconstruction. We describe the reconstruction of with three-dimensional (3-D) printed custom-made, anatomically designed, titanium alloy ribs hemi-sternum implant. To design implant manual threshold segmentation was performed to create 3-D virtual model patient's chest from sub-millimeter slice computed tomography (CT) scan data....
Herniation of the liver through an anterior abdominal wall incisional defect has rarely been described. An 81-year-old man presented to our surgical team with acute right upper quadrant pain. He had undergone coronary artery bypass grafting via a median sternotomy 7 years previously. Examination revealed gallbladder tenderness and non-tender epigastric hernia. Cholecystitis was confirmed on ultrasound. A CT scan knuckle (segment II/III) herniating midline defect.
A 26 year old man was referred to us with extensor pollicis longus dysfunction (EPL), and investigation revealed a fracture of the trapezium (left) carpo-metacarpal joint dislocation thumb. Operative exploration mechanical displacement tendon secondary as cause for EPL dysfunction. The lesion managed an open reduction screw fixation resulting in good functional outcome.
There are many surgical techniques for reconstruction of the divided or incompetent central slip. Most rely upon postoperative immobilisation to protect repair. We present our experience using a distally based flexor digitorum superficialis slip reconstruct and allow early mobilisation in five patients with intact preoperative intrinsic function. Post-treatment, mean recovery 65 degrees active extension was achieved, albeit at cost 22-degree loss flexion. suggest this relatively easy...
Hand infections are a common presentation to primary and secondary care. Early diagnosis appropriate management can often produce good functional outcomes, whereas delayed or inappropriate therapy risks permanent disability, amputation in extreme cases mortality. It be challenging know which patients treated the care setting require referral local hand surgery service, plastic orthopaedics as dictated by availability. There two main aims for this article. The first is provide framework...
A 70-year-old man was referred to plastic surgery with a progressively enlarging lesion his forehead. small nodule had been present since 30 years. The patient presented general practitioner at the time and reassured that this sebaceous cyst which warranted no further attention. As continued grow over intervening years, did not seek any medical attention concealed under cap. On examination, he 5×6×6 cm pedunculated, fleshy exophytic tumour on central forehead (figure 1A), nodules each...
