•We report a case of patient with acute myeloid leukemia (AML) presenting as sarcoma.•This bilateral adnexal masses was managed via total robotic hysterectomy salpingo-oophorectomy.•There are limited number reports ovarian occurrences that exist in the literature.•Myeloid sarcoma ovaries may present vaginal bleeding to dysmenorrhea, dysuria, and palpable abdominal mass.
