ABSTRACT
Importance Individuals who are amyloid-positive with subjective cognitive decline and clinical features increasing the likelihood of preclinical Alzheimer disease (SCD+) at higher risk developing dementia. Some individuals SCD+ undergo amyloid-positron emission tomography (PET) as part research studies frequently wish to know their amyloid status; however, disclosure a positive amyloid-PET result might have psychological risks. Design, Setting, Participants This prospective, multicenter study was conducted The Amyloid Imaging Prevent Disease Diagnostic Patient Management Study (AMYPAD-DPMS) (recruitment period: from April 2018 October 2020). setting 5 European memory clinics, participants included patients underwent amyloid-PET. Main Outcomes Measures Psychological were defined (1) related distress, assessed using Impact Event Scale–Revised (IES-R; scores least 33 indicate probable presence posttraumatic stress disorder [PTSD]); (2) anxiety depression, Hospital Anxiety Depression scale (HADS; 15 severe mood symptoms). Results After disclosure, 27 (median [IQR] age, 70 [66-74] years; gender: 14 men [52%]; median education: [13 17] years, Mini-Mental State Examination [MMSE] score, 29 [28 30]) had (IQR) IES-R total score (10 [2 14] vs 0 [0 2]; P < .001), avoidance (0.00 [0.00 0.69] 0.00 0.00]; intrusions (0.50 [0.13 0.75] 0.25]; hyperarousal (0.33 0.67] .001) than 78 amyloid-negative [IQR], 67 [64 74] 45 [58%], [12 MMSE score: 30]). There no observed differences between HADS (–1.0 [–3.0 1.8] –2.0 [–4.8 1.0]; = .06) [–2.0 0.0] –1.0 0.0]; .46) deltas (score after – baseline). In SCD+, despite small sample size, education lower disclosure-related distress (ρ –0.43; .02) whereas partner ( W 7.5; .03). No showed PTSD or depression symptoms follow-up. Conclusions Relevance bigger change, yet such change did not reach threshold for concern.
