ABSTRACT
Vaccine responses are decreased in solid organ transplant (SOT) recipients, and given the complexity of implementation, vaccination programs may be suboptimal. The actual burden vaccine-preventable infections (VPIs) among SOT recipients remains unclear. To assess incidence rate VPIs to evaluate whether at increased risk for specific compared with general population. This nationwide cohort study used data from Swiss Transplant Cohort Study on individuals who underwent May 2008 June 2019 (follow-up until December 2019) Federal Office Public Health notifiable population same period. Data were analyzed January 2021 2022.Solid transplant. The main outcomes following recipients: hepatitis A B, diphtheria, Haemophilus influenzae infection, influenza, measles, mumps, pertussis, pneumococcal disease, poliomyelitis, meningococcal rubella, tetanus, tick-borne encephalitis, varicella zoster virus infection. Age-adjusted standardized ratios occurred more frequently population. For factors associated occurrence explored morbidity mortality assessed.Of 4967 enrolled (median age, 54 years [IQR, 42-62 years]; 3191 [64.2%] male), 593 (11.9%) experienced least 1 VPI. overall VPI was higher that (30.57 per 1000 person-years [PY]; 95% CI, 28.24-33.10 PY) (0.71 PY). age-adjusted ratio (27.84; 25.00-31.00). In influenza infection accounted most episodes (16.55 PY [95% 14.85-18.46 PY] 12.83 11.40-14.44 PY], respectively). total 198 575 (34.4%) led hospital admission, a an death and/or graft loss (hazard ratio, 2.44; 1.50-3.99; P = .002). multivariable analysis, age 65 or older time (incidence [IRR], 1.29; 1.02-1.62) receipt lung (IRR, 1.77; 1.38-2.26) heart 1.40; 1.05-1.88) occurrence. In this study, 11.9% VPIs, than There significant these SOT, which highlights need optimizing immunization strategies.
