Plasma p‐tau181 is associated with brain amyloid deposition and predicts MCI conversion to AD

ABSTRACT
Abstract Background Plasma phosphorated tau 181 (p‐tau181) was found to change in concordance with amyloid PET and well before PET, suggesting that the secretion of p‐tau181 may represent a physiologic reaction β‐amyloid plaques. Our study aimed evaluate relationship between plasma whether predicts amnestic MCI AD conversion. Method We included 98 participants (43 demented 55 nondemented) who underwent florbetaben scan. Amyloid interpreted by visual assessment based on brain plaque load score (BAPL). The level were tested using Simoa assays. Another longitudinal cohort 37 patients at baseline followed up for least one year. ANOVA logistic regression used cognitive status. Cox examine conversion AD. Result Brain deposition 51% (21/43) participants, 11% (6/55) nondemented. significantly higher positive individuals (p<0.001). P‐tau181 alone discriminates status an AUC (0.889, 95% CI 0.826–0.951). In cohort, 49% (18/37) converted during follow‐up. A cutoff 2.545 able predict where Ptau181 has shorter free survival (median time month, 35.0 vs 14.0, p=0.029). Conclusion Higher associated could be surrogate blood‐based biomarker positivity prognosis MCI.
