Bile duct injury and use of cholangiography during laparoscopic cholecystectomy

ABSTRACT
Abstract Background Bile duct injury (BDI) remains the most serious complication of laparoscopic cholecystectomy (LC). A Swiss database was used to identify risk factors for BDI and assess effect intraoperative cholangiography (IOC). Methods Data patients from 114 institutions who underwent LC acute or chronic cholecystitis between 1995 2005 were in univariable logistic regression analyses. Results In total 31 838 patients, mean(s.d.) age 54·4(15·9) years, analysed. The incidence 0·3 per cent (101 patients), which did not change over time (P = 0·560). Univariable analysis revealed that male had a higher (0·5 versus 0·2 female patients; P 0·001), as whose operation lasted at least 150 min (1·1 0·1 operating less than min; < 0·001). Logistic confirmed sex (odds ratio (OR) 1·89, 95 confidence interval 1·27 2·81) prolonged surgery (OR 12·60, 10·87 23·81) independent factors. Comparison groups with without showed no difference (both cent; 0·755) BDIs missed during (10 8 0·737). Conclusion Male are LC. Frequent use IOC does seem reduce number injuries surgery.
