ABSTRACT
Abstract Objective Audit the crown‐rump length (CRL) measurements taken at 11 to 13 weeks scan, using operator‐specific median multiples of (MoM) for pregnancy‐associated plasma protein‐A (PAPP‐A) and free β ‐human chorionic gonadotropin ( ‐hCG) plots, identify deviations potentially related a systematic CRL bias. Methods Study population included consecutive singleton pregnancies undergoing first trimester combined screening, scanned by sonologists with least 100 scans, during 2011 2012 period. Previously described plots PAPP‐A ‐hCG MoM points, their 95% confidence intervals circles, in relation expected deviation line were used. These have been modified adjust sonologist‐specific variation each millimetre bias according gestational ages both blood sampling ultrasound. Results Twenty‐eight performing 9472 scans included, accounting 36% 77 70% 643 initially considered. Mean age was 10 + 2 12 4 Fifteen (53%) did not demonstrate any bias, (36%) present significant underestimation, being above mm 6 (21%), 3 (11%) observed could be explained Conclusions In more than NT measurements, are able Systematic underestimation 1/5 them. © 2016 John Wiley & Sons, Ltd.
