ABSTRACT
We measured serum levels of human glandular kallikrein 2 (hK2) in patients treated with radical retropubic prostatectomy (rrP) for clinically localized prostate cancer (PCa) a total PSA (tPSA)-level below 10 ng/ml to investigate whether hK2 can be applied preoperatively distinguish organ-confined (pT2a/b) from nonorgan-confined (> or = pT3a)-PCa more accurately than PSA. Further, we evaluated hK2, free- and tPSA-concentrations all pathologic stages PCa.161 samples men scheduled rrP were collected 1 day before surgery prior any prostatic manipulation. Pathologic work-up revealed > pT3a-PCa 48 pT2a/b-PCa 113 patients. HK2-levels using an immunofluorometric assay analytical sensitivity 0.5 pg/ml, functional 5 pg/ml insignificant cross-reactivity (< 0.005%). Total (tPSA) free (fPSA) commercially available which calculated %fPSA algorithm that combined PSA-levels [hK2] x [tPSA/fPSA]. Means, medians, ranges pT2a/b vs. >/= stages. Statistical significance differences was Mann-Whitney-U Kruskal-Wallis tests. Calculation receiver-operator-characteristic (ROC) curves performed [tPSA/fPSA] tPSA compare diagnostic performance.A mean level 6.12 not significantly different (P 0.366) 5.78 pT2a/b-PCa. Also, there no statistically fPSA 0.947) (0.292) these two groups. By contrast, hK2-level 80 0.004) 120 as shown by Mann-Whitney-analysis Moreover, the lower 0.0004) pT3a-PCa. areas under curve (AUC) receiver-operator-characteristics demonstrated AUC (0.64) larger (=0.68) 0.007) compared (0.55). Furthermore, Test highly significant correlation stage 0.008) 0.0015) concentration 0.296). Also at tPSA-levels 10-20 ng/ml, hK2-levels close 0.051) those pT3a-PCa, while tPSA-range 0.002) pT3a0-PCa.Highly enable powerful predictor disease cancer, especially PSA-range ng/ml. As such, are important clinical consequences application adequate treatment patients, i.e., option nerve-sparing surgery. (c) 2001 Wiley-Liss, Inc.
