Abstract Purpose The present study aimed to examine the association between adherence Japanese diet and subsequent risk of all-cause cause-specific mortality using a large-scale cohort from settings all over Japan. Methods We analyzed data 92,969 adults aged 45–74 years, covering 11 public health center areas nationwide. collected dietary information validated 147-item food frequency questionnaire. Adherence consisting eight components (high intake rice, miso soup, seaweeds, pickles, green yellow vegetables, fish, tea; low beef pork) was assessed 8-item Diet Index (JDI8) score, with scores ranging 0 8. Cox proportional hazards model used estimate hazard ratio (HR) 95% confidence interval (CI) for mortality. Results During median follow-up 18.9 we documented 20,596 deaths. A higher JDI8 score significantly associated lower cardiovascular disease (CVD) multivariable-adjusted HR CVD highest group (score 6–8) versus lowest 0–2) were 0.86 (95% CI 0.81–0.90, P trend < 0.001), 0.89 0.80–0.99, = 0.007), respectively.
