ABSTRACT
Abstract Heart failure (HF) is a leading cause of mortality and hospitalization in the elderly. However, data are scarce about their response to device treatment such as cardiac resynchronization therapy (CRT). We aimed evaluate age-related differences effectiveness CRT, procedure-related complications, long-term outcome. Between 2000 2020, 2656 patients undergoing CRT implantation were registered analyzed retrospectively. Patients divided into 3 groups according age: group I, < 65; II, 65–75; III, > 75 years. The primary endpoint was echocardiographic defined relative increase 15% left ventricular ejection fraction (LVEF) within 6 months, secondary composite all-cause mortality, heart transplantation, or assist implantation. Procedure-related complications also assessed. After implantation, LVEF showed significant improvement both total cohort [28% ( IQR 24/33) vs. 35% 28/40); p 0.01)] each subgroup (27% 34%; 0.01, 29% 35%; 30% 0.01). Response rate similar (64% 62% 56%; = 0.41). During follow-up, 1574 (59%) died. Kaplan–Meier curves revealed significantly lower survival older (log-rank 0.001). cumulative complication rates among three age 28% 24%; 0.15). Our results demonstrate that effective safe elderly for young ones. present suggest with appropriate indications benefit from long term, regardless age.
