ABSTRACT
Abstract Background Obstructive sleep apnea (OSA) is the most common organic disorder. Nocturnal positive airway pressure (PAP) therapy first-line treatment, although effects can only be expected with regular and long-term use. In view of increasing OSA prevalence, interface problems in existing care are becoming apparent, leading to inadequate long waiting times for diagnosis treatment. This also has an impact on treatment adherence support. Hypothesis A telemedically supported patient pathway leads timely needs-based initiation improves adherence. improved quality life. Patients methods a prospective randomized controlled multicenter study, patients clinically suspected treated according guidelines or without help telemedicine (telemedicine control groups). total n = 4448 will included. After guideline-compliant initiation, followed up 12 months, whereby group receive additional telemedical medicine For 371 group, it investigated whether home-based PAP under polygraphic expedient case high clinical pre-test probability, clear diagnostic polygraphy findings, low comorbidity burden. The new services reimbursed participating medical service providers using project funds. addition evaluation, health economic analysis secondary data from insurance funds (TKK, BARMER, DAK, IKK classic) carried out. Primary outcome sleepiness-related life measured standardized functional outcomes questionnaire (FOSQ). Secondary endpoints patient-reported (daytime sleepiness, satisfaction), number treatments, adherence, cost effectiveness utility forms care. satisfaction evaluated. Results results study at end third quarter 2025. Registration DRKS00027895
