ABSTRACT
and importance: Congenital diaphragmatic hernia (CDH) is a condition characterized by defect in the diaphragm causing protrusion of abdominal organs into thoracic cavity. Comprehensive management, definitive surgical procedures postoperative care are able to significantly reduce morbidity mortality post-herniorrhaphy patients. Here, we reported case post herniorrhaphy pediatric patient with challenge extubation.A 4-month-old girl chief complaint respiratory distress from was admitted our Pediatric Intensive Care Unit. The diagnosis confirmed through small bowel follow-through radiological examination. Definitive treatment laparotomy were then done. special technique for extubation applied. Currently, survives without any sequelae awaiting stoma closure.Extubation removal an endotracheal tube when it no longer needed. In mechanically ventilated patients, can be performed two ways, either using tracheal suction catheter (TSC) or positive pressure breath (PPB) techniques. Studies show that PBB has better outcomes compared TSC technique. However, more commonly done medical professionals. We used PPB because there recurrent atelectases left lung.This report illustrates congenital VACTERL association. Moreover, several options techniques procedure CDH.
