Inside-out meniscal repair is considered the gold standard for reparable tears of medial and lateral menisci despite recent popularity all-inside devices. Accurate suture passage required to perform a stable as well prevent inadvertent neurovascular injury from needles. Placement deep soft-tissue retractor necessary identify retrieve these needles prior tying sutures. Several authors have recommended placement this in interval anterior gastrocnemius muscle belly above semimembranosus tendon. However, we noted that often pass distal when it placed owing reorientation joint line occurs with knee relatively extended position during placement. We describe modified technique which inferior semimembranosus, puts directly needles' trajectory. This modification makes inside-out safer more efficient.
