ABSTRACT
Background: Salvage living donor liver transplantation (LDLT) is technically challenging due to adhesions caused by previous resection (LR). The purpose of this study was assess the technical feasibility LDLT after major hepatectomy and determine whether there a difference in surgical outcome according type LR received previously. Methods: We retrospectively reviewed medical records 83 patients who underwent from December 2004 June 2022, at Asan center. Results: Among patients, 74 salvage for recurrent hepatocellular carcinoma. Of 44 right hemihepatectomy (RL group) 30 left (LL prior LDLT. operative time longer (mean ±SD, 861.05±151.02 vs. 737.67±94.61 min; p = 0.000) intraoperative RBC (median (IQR), 8 (2, 19.5) 0.5 (0, 6) units; 0.003) transfusion requirements were higher RL group. In group, postoperative complication significantly (54.6% 30.0% p=0.037). However, no detected regarding survival rate. 1-, 3-, 5-year overall rates 93.2%, 80.5%, respectively, 96.7%, 85.5%, LL group (P=0.789). Conclusion: can be safely performed with recurrence or deterioration function even hepatectomy. demanding procedure, it should experienced transplant surgeon.
