ABSTRACT
Pulmonary complications after bariatric surgeries are rare but usually serious. They often occur early surgery the presentation might be delayed for several months. Gastropleural fistula is extremely and has been reported in a very small number of patients post sleeve gastrectomy gastric bypass. A 37-year-old lady presented with left sided pleural effusion empyema 2 years single anastomosis bypass surgery. She was found to have large gastropleural managed by surgical repair conversion decortication affected pleura. That resulted significant clinical improvement resolution empyema. complication should considered who present chronic or recurrent pulmonary infections.
