Trapped fourth ventricle: A case report and review of literature

ABSTRACT
Trapped fourth ventricle (TFV) also known as isolated (IFV) is a rare clinico-radiologic entity with only few cases reported in the literatures. The aim of this article to present first case condition our center and highlight challenges arriving at clinical diagnosis treatment resource limited setting. An 18 months old girl who had ventriculoperitoneal shunt insertion for post meningitic hydrocephalus 4 earlier presented restlessness, ataxia, fever inability control her neck one-week duration. On examination she was restless retro-colis Glasgow Coma Scale (GCS) score 11/15 (E4V2M5). She an associated facial abducent nerve palsies global hypertonia, hyper-reflexia muscle power 3/5. initially treated infection malfunction. However, series CSF analysis were within normal limits culture yielded no growth microorganisms. A CT scan brain which ordered delayed 10 days due financial constraints. revealed trapped slit lateral third ventricle. emergency inserted on left because pre-existing supratentorial right. did well after surgery discharged 10th postoperative day. doing 12 surgery. TFV may occur VPS post-meningitic hydrocephalus. This diagnostic dilemma. Insertion second be option
