ABSTRACT
Kommerell diverticulum is a very rare congenital defect of the aortic arch associated with aberrant subclavian artery. It can present signs dysphagia, chest pain, or distal embolization in upper limb. We case large artery aneurysm male patient pain unknown origin and hypertension. There was an incidental finding wide mediastinum on X-ray had difference systolic blood pressure both arms. A right thoracotomy incision used to successfully excise reconstruct Patient recovery uneventful. Endovascular approaches are also alternative conventional open surgeries treatment diverticulum. should be considered differential diagnosis non-cardiac pain. simple investigation such as make these patients. Coarctation related ASA might not always true coarctation. repair selected cases, but it needs further randomized control trials.
