Pediatric oral food challenges in the outpatient setting: A single-center experience

ABSTRACT
BackgroundOral food challenge (OFC) is the criterion standard for diagnosing allergy (FA). It important to have parameters aid in selecting ideal OFC candidates.ObjectiveWe sought characterize outcomes and predictors of OFCs common allergens.MethodsWe completed a retrospective chart review all IgE-mediated FA performed at Duke University pediatric clinics from June 2017 through May 2022. Patients were deemed eligible milk, egg, nut if testing revealed specific IgE level not exceeding 2 kU/L skin prick test (SPT) resulting wheal size 5 mm. Different followed candidates baked challenge.ResultsA total 663 conducted on 510 patients (59% male). The most foods challenged peanut (26%), plain egg (23%), (8%), milk with pass rates 84%, 88%, 62%, respectively. Of who failed OFC, 84% had objective symptoms, 23% multisystemic reactions, 15% required epinephrine. Although presence personal or family history atopy prior was associated outcomes, anaphylaxis (regardless trigger) increased risk failure.ConclusionAlthough there are no established consensus guidelines, our study provides benchmark illustrating that cutoffs SPT finding mm result failure rate approximately 13% nonbaked nuts. high likely related selection bias, but results illustrate low negative predictive value ovomucoid.
