ABSTRACT
The horizontal transfer of antibiotic resistance genes in Gram-negative bacteria, mainly through plasmids, is one the greatest concerns for health systems worldwide and has been a growing threat hospitals related to healthcare-associated infections by multidrug-resistant bacteria. Here we present p henotypic genomic characterization KPC-2 MCR-1.27-producing Klebsiella pneumoniae strain isolated from paediatric patient at an oncologic hospital Belém, Pará State, Brazilian Amazon region.Antibiotic susceptibility test, whole genome sequencing, silico analysis were used characterize bacterial isolate (IEC48020) received Evandro Chagas Institute. The was resistant carbapenems, colistin, polymyxin B, several other antimicrobials susceptible vitro just tigecycline, classified as extensively drug-resistant phenotype. Genomic revealed IEC48020 belonged sequence type 11, clonal complex 258 high-risk clone presence eight two them harbouring mcr-1.27 blaKPC-2 genes, virulence-related encoding yersiniabactin, phospholipase D, traT genes. The dissemination bacteria with high disseminating plasmids containing last resource antibiotics treatment options healthcare system demands efforts surveillance epidemiological research better knowledge actual situation system, especially region, Brazil.
