SummaryBackgroundHereditary hemorrhagic telangiectasia (HHT), is a very rare vascular disorder, characterized by arteriovenous malformations of various types. High cervical dural arteriovenous fistulas are extremely rare and usually manifestate with progressive myelopathy.Case presentationWe present neurovascular imaging in a patient suffering from a cervical intradural extramedullar and intracranial arteriovenous malformation who presented with repeated transient ischemic brainstem attacks. This case illustrates the hemodynamic and anatomical information derived from multimodal neuroimaging procedures. The chest CT detected a pulmonary AVM.ConclusionEvaluating younger patients with acute brain ischemia, HHT has to be kept in mind. Pulmonary AVM often leads to TIA or stroke due to paradoxical embolization.
